Update as of 4/23/20


We are happy to announce that a directive from the Utah Department of Health and the Governor’s office released on April 22nd now allows “healthcare providers” (including dentists and orthodontists) to resume “elective procedures” while implementing heightened office/clinical protocols. This means we will begin seeing patients in office beginning Monday, April 27th!

It is very important to us (as well as you) that we all do our part to keep our community safe moving forward. For this reason, a visit to our office will be different than what you have experienced previously. Before outlining in-office expectations, we want you to know we are aware each patient’s risk tolerance is different. If you do not feel comfortable coming into the office on your next appointment date, please feel free to call us so that we can schedule another appointment time on a date/time that fits your comfort level.

For those patients who will be visiting our office over the coming weeks, please become acquainted with how your in-office visit will be conducted until further notice. We have made a fun video outlining the new process. You can view the video by clicking here.

Before arriving to our building, we ask that you fill out the online health questionnaire and consent form. If the form hasn't been filled out prior to your appointment, before entering Cook Orthodontics, we will have the patient (or a legal guardian) fill out the questionnaire/consent. 

Until further notice, we are requesting  ONLY the patient enters our office. Parents and family members are free to wait outside (of the building) or in your vehicle. If you have a question about a patient’s orthodontic care, Dr. Cook will be happy to discuss over the phone.

If a patient is receiving an initial orthodontic evaluation, legal guardians should feel free to accompany the patient to the appointment (but will go through the same process outlined above). If a current patient requires a parent/sibling attend their appointment with them, please contact our office so we can make special arrangements to have the patient scheduled in a separate treatment room.

As the safety of our community, patients and staff are our utmost priority, we are following the recommendations of our national, state and local governing authorities and discontinuing non-emergency in-office patient visits beginning Friday, March 20th, 2020, to help contain the spread of COVID-19.

We will contact you to reschedule your appointment when it is recommended that you can safely be seen. Please do not hesitate to contact us in the meantime with any questions or emergencies. We will be offering virtual triage through Zoom Healthcare Teledentisty to handle this. Our office will be contacting you for future virtual exams. Dear Patients and Families,

As a valued patient, or family member of a patient, I appreciate the trust you place in me and the Cook orthodontic team. We truly strive to provide the best orthodontic and healthcare experience possible to all our patients. I also want to assure you that the well-being of our employees, and you, our patients, is our top priority.

As a healthcare provider, stringent health and safety procedures are ingrained in our culture and operations as it has always been our practice to follow the standards of the CDC, OSHA and the American Dental Association. As a team we have reviewed these standards and, as always, will diligently adhere to the highest health and sanitation standards.

In response to the heightened concern regarding COVID-19, I want to personally inform you of the additional health and safety measures we will be taking to reduce risks and mitigate spread of the virus. In an effort to meet and/or exceed CDC recommendations, we have added the following precautions:

  • We will be wisely scheduling to assure that as few patients as possible will be in the office at a given time.
  • All areas of the office adhere to recommended “social distances”
  • We also have a private treatment room that we can reserve for your care if requested (please request 1 day prior to your visit)
  • Ensuring that our employees are healthy by conducting brief interview and temperature checks each morning before beginning work


We also ask for your help in keeping our office clean and safe for patients and families and ask that you please do the following:

  • To limit person to person contact, only scheduled patients will be allowed into the office. We will make exceptions for medical and behavioral issues. Treatment progress can be discussed over the phone. Parents and family members should remain in the car.
  • Call and reschedule your orthodontic appointment if you have any “sickness” symptoms (especially coughing or fever) or you have been exposed to someone who is suffering from those symptoms.

During unique times such as this, we believe that accurate information as well as hopeful smiles are equally valuable in combating some of the anxiety that is prevalent. We will continue to see patients for their regularly scheduled visits, and do not have plans to close our office unless told to do so by our dental/governmental governing bodies. We are at the ready to communicate via text or phone regarding treatment questions, concerns and scheduling questions.
