Nominate a deserving friend or family member to receive FREE orthodontic treatment from Cook Orthodontics. We’ll select two recipients from community nominations.

How It Works
- Send your nomination to info@orthofamily.com by December 20, 2025.
- Our team reviews all submissions. Dr. Cook and staff will choose the winners.
- Nominators are notified first. We’ll email the person who nominated the winner(s).
- Gift the certificate: we provide a “Certificate of Free Ortho” to the nominator to present as a holiday gift.
What to Include in Your Email
- Your name and phone number
- Nominee’s name and age
- A brief story explaining why you’re nominating them
- Any relevant details that could help our team understand their need
Eligibility & Details
- Open to community members; minors require parent/guardian consent.
- Winners will receive comprehensive orthodontic care as determined clinically appropriate by Dr. Cook.
- Two winners will be selected.
- Care must be initiated within the timeframe specified on the certificate.
- No purchase necessary; not transferable or redeemable for cash.

How are winners notified?
We first contact the nominator by email. The nominator then receives a “Certificate of Free Ortho” to present as a gift.
What does the free treatment include?
The scope of treatment is determined after a clinical evaluation and will be clearly explained before care begins.
Can I nominate more than one person?
Yes. Please send a separate email for each nominee so our team can review each story individually.
